What steps are taken to get the A off my CPC?

I've been trying to browse the aapc site on how to get the A off the CPC. Obviously, I need a years experience, which I have, but it is between 2 different companies. Are there any forms I need or do the employers just have to write a general letter? Thanks for your help.
 
You need to have your employers write a letter (probably on company letterhead) verifying that you have held a coding position with their company and for how long. You mail it or fax it to the AAPC.
