Hi All,
I have a new visit in which the HPI is for preventive Visit BUT after the Physical Exam it was found out the patient has- Dermatitis- and was given RX..SO;; if I wil code for 99203 but the HPI does not have 4 Elements..so I don't know of what CPT will I keep PREV? OR NEW VISIT?

It is not unusual for a doctor to prescribe something as a result of a preventive visit. If the problem is limited, it is considered to be a part of the preventive workup. From the little information you have given, it seems a preventive visit was the intent, so you should leave it as is as long as services performed meet this level of CPT code.
