A question for those of you billing psychiatric CoCM: The patient intake is completed in October but consent is not obtained until early November; the patient then has follow-up care in November. No billing is submitted for October because no consent was on file but the November billing received is a subsequent month 99493. 99493 is denied by the payer because they've received no prior initial billing.

How do you handle this in your practice? I think we should be billing 99492 in November instead of 99493, because it's the first billable service after the consent was obtained. We're having internal discussions about this but I'm curious what others are doing.
