When to use history of for DVT

How would I code a patient that has "recurrent DVT"? He is on lifetime anti coagulation, but as far as I can gather from my provider's note, he doesn't have the DVT at this time. Would I code it as a current condition since he is still on anti-coagulation, or hx of because he no longer has the DVT? :confused:
