Can anyone tell me where I can find the guidelines or rules for coding a diagnosis from the postoperative diagnosis or the pathology report findings? (such as benign tumors). Through my coursework in AAPC, I have been coding from the pathology report if it gives more specific details about the diagnosis. (Ex; postoperative diagnosis says removal of cyst, but pathology says it's malignant).

I need to find the guidelines for using this methodology. Any help would be greatly appreciated!

postoperative vs pathology report

Cyst doesnt mean it is completely benign. According to the facts of cysts :The prognosis for most cysts is good; a few cysts that contain malignant cells have a more guarded prognosis.

So the pathology report is showing it is malignant. However it is the removal of cyst in the postoperative diagnosis..
