Where does the "shaft" begin/end in a long bone for coding purposes? Does anyone have any advice or doc they could share? Case example: Distal Radius and ulna fx. This dx is all I'm given on a reg basis. I assume "both bone fractures" means the shaft, but they don't pay me to assume. I've been taught that if the fx is above the growth plate, then it's in the shaft (for CPT coding) and then I use the approp dx for the distal fx.
