Wiki Which diagnosis codes to report for inpatient?

When a patient is hospitalized. On the discharge summary a provider will document primary and secondary Diagnosis? Do you only submit the dx's that were addressed during the hospital stay or all of them? I also have a provider that for observations discharge they list all active dx's , do we only report what the patient was being seen for? Any guidance would be great and if there is some "rule" written somewhere, I have been looking in my ICD 10 book and can not seem to find anything on it. Thanks in advance for anyone's help with this.
 
On an inpatient you add all the diagnoses that were investigated during the stay. Even if they are ruled out. That is one major difference from outpatient coding.
