Wiki which dx needed for pathology?

The doctor performs a colonoscopy for colon cancer screening for pt with family h/o colon cancer. She finds a polyp which she removes by cold forceps and diverticulosis and notes external hemorrhoids. When she sends the specimens to pathology does she need to include only the pertinent icd9 (211.3, V76.51, V16.0)? Or does she also need to include diverticulosis and external hemorrhoids?

Our path reports have a clinical history from the doctor and a note about the specimens sent. Then our docs (the pathologists) note their findings. The reports we receive don't have ICD9 codes on them. So I guess I'm not sure what you're asking.
