Benign hypertensive heart disease due to systolic heart failure
 
This would be coded as 402.11 since the physician linked the HTN to the heart disease and that will go first listed, the heart failure then is listed secondary per the instructions in the guidelines as well as the instructional note to "use additional code to specify type of heart failure if known" so for your scenario it would be 428.20
