MD sees the patient and after evaluation determines that the patient might benefit from an injection (steroid) to be given same day. Order is put in. PA signs the consent and gives the injection. Who should the injection be billed under? We are not billing for an E/M as no other issues were addressed.
 
For us, it would depend on the carrier. If the carrier follows incident-to, this would be billed under MD, since the MD saw the pt & established the plan of care and is physically present. If not, then under PA.
