Wiki Whichi modifiers on lesion removals

I have four lesion removals I billed to Medicare as follows:

11404
11302 59
11301 59
17000 59

The adjusted off 11404, and paid on the other three. I called to get help and they told me a modifier was needed, but that they couldn't help determine which modifier. My coworkers and I don't know what would be appropriate here. PLEASE HELP!!!
 
modifiers

If you check the CCI edit it will tell you which codes to put the modifiers on . In the scenario that you had, it should be billed as 17000, 1140459, 1130259, 1130159.
