I'm hoping someone can offer some assistance with this, as I'm having trouble finding anything in writing from CMS or my MAC Novitas.

Who is allowed to dictate an E/M service that an MD has performed? In this particular issue I'm looking into, the service was performed in an inpatient setting by one of my MD's. But was dictated by an employee of the facility the patient was admitted to. I've found all the guidance on scribes and such, just can't find anything definitive about who is allowed to dictate. My gut instinct says that any provider should dictate his/her own services, but I need proof!!

Appreciate any info/resources anyone can offer.
