Who can resolve outdated diagnoses? What are the guidelines for this?

When someone is living in a SNF and have had temporary dx such as UTI's, pneumonia, fractures, respiratory infections etc, how do we resolve these diagnoses? Do we need the PCP to document that they are historical? Or, if they are no longer being treated can we resolve the active diagnosis and give them a "history of" dx?
