Depends on who the publisher of your book is. My Ingenix version has in it.

It is in the CPT book I have for 2012, and the reason it was not in the 2011 book is it was not created until after the 2011 book was already printed and out the door.
I am not sure what version of the book you have, but mine is 2012 and I have also looked at the Ingenix version and it is in there also. I have not yet seen the PMIC version.

I have the AMA Professional Edition of the 2012 book...33 is in Appendix A, but not listed on the back of the front cover with the others.
