Does anyone know which form is used for billing a procedure performed at an ASC for a CA Work Comp case? Would you use the CMS 1500 as Medicare does or the UB 04 that we use for all others? Thanks very much for any help!
 
I would think it depends on the payer,we are in MO and some of our W/C claims go on UB's, some on 1500's. We have a form that the scheduler fills out when she calls to verify the claim information, and she verifies what type of form they prefer at that time.

I found that California has no guideline for which form to use. In talking to payors I found that most prefer the UB 04. Thank for responding! :)
