Good morning!

I am having a problem figuring out how to code a workers comp claim. This is my first coding job and this one has me stumped.

A woman cut her finger with scissors while working at a salon (Sports Clips). She had a small, simple closure laceration, under 2.5 cm.

Here are the codes I used:

99455
12001-F2
S61.213A

What do I do for place of occurrence and activity code? I can't find anything that relates to cutting finger or doing so while at work. Also, do I use a modifer (-25) on 99455 because a procedure was performed?

Hello,

I would modify the e&m code with 25. Also see dx codes y28.8xxa, y92.513, and w27.2xxa for reference to place and type of injury and decide if these fit your documentation.
