Would this count as status of 3 chronic conditions

Patient with a history of medically complicated obesity, HTN and type 2 diabetes. Would this count as status of 3 chronic condition?

I personally wouldn't count that as the status of 3 conditions. Yes, there are 3 conditions but simply stating "medically complicated" before listing the conditions seems very vague. I wouldn't consider that as addressing the status of the conditions.
 
If that's all you have and since there is no real "status' defined in your statement, no, I would not count that as "status" of 3 conditions. That's more of a history statement.
 
Thank you. So really you can not got these as part of the medical decision make for number of diagnosis since they where not address right?
