Have a note that specifies pt has a Stage 4 Pressure Ulcer - excisional debridement done to subcutaneous level. The documentation supports the 11042 with L89.214. My question is...is it common within wound care to have a Stage 4 but only do a debridement to the subcutaneous level?
I've checked to make sure that they didn't pull information from the previous note and forget to amend it for the current visit.
I've looked into this but can't find what I need. EncoderPro is not showing dx code L89.214 under procedure 11042. This is the first one I've run across where there is a conflict between the stage and the debridement level.
