Hello All,

As we move closer to ICD-10 I have been discussing the need to document depth of ulcers to my physicians. Here is the issue....the choices to come to a specific code are breakdown of skin, fat layer exposed, necrosis of muscle and necrosis of bone. How is everyone interpreting the word "necrosis?" This is the meaning of necrosis: "the death of most or all of the cells in an organ or tissue due to disease, injury, or failure of the blood supply." Just because an ulcer probes to muscle or bone doesn't necessarily mean that there is necrosis of the muscle or bone.

I would like professional opinions please. Chime in please I need all the help with this that I can get.
