Good Afternoon - I have an Indiana physician that bills a lot of shave excisions to WPS Medicare we used to used dx code 239.2 as the physician documents the lesions are pigmented and irregular. He does not always send them for path. The WPS policy does not included ICD-10 code D49.2 for unspecified nature of skin neoplasm. Coder has been submitting L98.9 because he does not specify a dx code, which is not on the list of medically necessary ICD-10 codes either. Needless to day they are being denied. How are you all submitting in this situation.. ? Your assistance is greatly appreciated. I try to appeal stating the lesions were pigmented and irregular, but they are still denying.
