X-ray ankle: 73600 still denied due to modifier several times.
we added LT T4 for specifically w/ DX M79.675 , S99.912A
Do I need to add 26 or/and 59 instead? other Insurance paid but the other is not... please help me.

Why would a toe pain dx be on an ankle XR? Why would you append a T4 to an ankle XR? A toe modifier makes no sense on this code. Maybe they took the XR to check the ankle due to the toe pain but the modifier doesn't align.
 
because our another biller said pt did Xray ankle ( it was covered toe and upper foot) lol? so if we remove T4 what modifier would you recommend?
or I was thinking 73620 is better with T4. any advise for me?? Thanks.
