I know there is no CPT code for the removal of an X STOP, but how would I code if the surgeon removes the device and then at the same level places non segmental instrumentation with a one level fusion? Can I code 22849? or 22899 and 22840?
 
Per 2012 guidelines the removal would not be coded only the 22840 would be coded. Basically any instrumentation removed and replaced by new or modified instrumentation, the insertion is only coded.
