Is there a list somewhere of which treatments (chemo drugs) require which z codes? or do all of them regardless of what it is used for, require a z code?

Coding guidelines only tell you how to assign codes based on provider documentation, not based on the type of treatment performed, so you won't find any generic list in the guidelines of what codes are required for what drug. Some payers may publish requirements for payment and so you might want to try reviewing the reimbursement policies of the payers you most frequently work with to try to find something like this.
