Is there any guidance on what needs to be documented when applying the Zio Patch? Or, how does your office document for 93246?
 
Usually by the nurse doing a nurses note stating the monitor was applied or if it was mailed etc or if the MD saw the patient that day, the physician states in the note that the monitor would be placed today. If the patient comes in separately on another DOS then schedule a nurse visit with the note that monitor was placed.
