Question: Patient had a urethral sling, and six months later, she complained in office about the mesh. The ob-gyn trimmed the mesh (Mesh revision). Any suggestions on what CPT® code to use?

Answer: That will be an unlisted procedure when performed in the office, so you should report 58999 (Unlisted procedure, female genital system [nonobstetrical]). The comparison code in this setting for insurer payment might be 10120 (Incision and removal of foreign body, subcutaneous tissues; simple), which has 4.32 relative value units (RVUs). The ob-gyn used scissors to trim the mesh. This makes it much less complicated than the work involved in a mesh revision (57287 (Removal or revision of sling for stress incontinence [eg, fascia or synthetic]) which also requires the surgery to be performed in the a facility setting.
