My practice was "advised" years ago if a physician is doing an H & P for procedure and he is addressing issues above and beyond the H & P He must have a separate note. Do not use the same note for H & P and visit. Even if the above and beyond issue is addressed in the H & P. I believe it was a Blue auditor but can not be sure . Anyone else hear of this

It is correct that two separate notes would be needed. So there's documentation "A" for the H & P and documentation "B" for the unrelated problem(s). If documentation A is mixed with documentation B, there would be no way to level each portion. There would also be no way to tell what amount of work was spent for A and what work was spent for B. If the provider does work for documentation A, he/she can't also count that a second time for documentation B. By separating the notes, you have a clear distinction between the work for A and the work for B, thus you can level appropriately.
