I am getting ready to take my cpc exam for the 3rd time , can someone clarify for me the way I was taught
1st dx condition
2nd signs and symptoms
3rd and then recheck with alpha or tab list
 
Not sure what you are asking, sorry. Only a verified diagnosis is coded. If the doctor is not able to identify a specific diagnosis, then you code the signs and symptoms. Signs and symptoms are never coded when the doctor has been able to identify the medical condition that is causing them. You always start in the alfa section and then verify the code in the numeric. In the numeric verify that the code is complete, many are not. Also look for coding guidelines given. There are coding instructions for entire sections, so make sure you go back and read those as they could direct you to code in a different section. I hope this helps.

So i'm correct the condition if dr is able to identify them . If not signs and symptoms. so you need to pull the condition or signs and symptoms go to alpha index or tabular then go to your dx code?
 
Yes. And make sure the code is complete, and watch for coding instructions that can direct you to another diagnosis code in another section, or to code another medical condition first.
