What is the correct diagnosis code for a pregnant woman who presents with a history of herpes at 37 weeks without current breakout? Patient was started on started valacyclovir 500 mg twice daily to prevent herpes outbreak during labor.

I'd code it as Z34.03, then Z3A.37 and Z83.1 since she is 37 weeks pregnant and PAST herpes in her medical history and not current problem. 026.4 shows Preg with herpes on skin if current. I hope I helped you. However if pt has ongoing chronic condition pick a dx defines this too. (ie Prego with DM select see dxO24.)

Inactive Herpes (no outbakes) O98.311
Active lesions O26.4-
History Z codes explain a patient's past medical condition that no longer exist and is not receiving any treatment.

O26.4 is for herpes gestationis. It is a skin disorder not related to herpes, eg A60. Pemphigoid gestationis (PG) is a skin condition that usually presents in the second trimester of pregnancy; however, it may occur at any time during the pregnancy, as well as in the days post-partum. PG was formerly called herpes gestationis, but it not related to the herpes virus.

Z83.1 is for a family history of infectious/parasitic diseases. Z86.19 is the code for the personal history.
