Wiki 17000 and 17003- with and without an E/M

Hi,

Looking for help with a cryo question. I have been getting edits from my billing department about CPT codes 17000 and 17003- it doesn't seem to matter if there is an E/M level or not. I am getting "Viewed CPT 17000 on Vitalware and see it's paid separately." It sounds like they are looking for a modifier for the 17000 but that doesn't sound right to me. Any suggestions/ideas/solutions would be greatly appreciated! Thank you!
 
CPT code 17000 is for the first lesion
CPT code 17003 is reported with units for each lesion 2-14.
if there are 15 or more lesions then CPT code 17004 should be reported with one unit of service.
 
there was a cci edit that was almost forcing you to append a 59 modifier on 17000 and if there was any other destruction type things done same dos they want a 59 on the 17003 too when you do a quick check in derm coder it was telling you to put 59 on them as well it is very odd and nothing we used to do but doing this got the claims paid, as a few carriers were rejecting without the 59- hope that helps
