Wiki 17110 denial by medicare

pt dx is sebhorric keratosis. had bothersome itchy lesion removed (17110) but medicare denied because wasn't medically necessary. any suggestions?
 
did you use 702.11 for dx. This is the dx to use for the irritated SK - be sure not to use 702.19 that is for asymptomatic.
