I billed 28805-rt and 28805-lt, it came back denied. Should it have been just one 28805 with a 50 modifier or the way i did should be correct?
 
Your diagnosis could be the issue here

Some insurance companies like bilateral procedures billed with -RT & -LT like you did. Others prefer -50. You will just have to keep track of them to determine their preference. However, in this case I think the most likely reason for the denial is your diagnosis code. Without knowing the reason behind the denial, there is no way to tell. The first thing that I would do is check to see if you can bill with a more specific diagnosis.
