Wiki 88305 or G0121, G0105, G0104 for Medicare Patient?

Which Pathology CPT codes do you use to Bill Medicare patient colonoscopy? 88305 or G0121, G0105, G0104?

88305 is utilized where I work for a specimen acquired during a colonoscopy. Sometimes multiple specimens are sampled by the surgeon and the subsequent charge or charges would have XS modifier for separate specimen.
