I am also having a hard time getting paid for this code. For the 99058 are you adding a modifier along with this code if there is a E&M?

You do not need a modifier and the code can never be listed by itself. It is always listed as a secondary CPT code. Some payers pay and some do not. I have been paid on appeal every time though. You must support that the visit was emergent and disrupted the office routine.
 
Hi my provider is Out of network to most commercial insurance and frequently billing for 99058. we never got paid for this line and other services are being denied too if the patient plan is in-network only. Does anyone here experience getting their claims enhance as INN for billing an emergency office visit code?
