Question 99406 modifier with AWV

I am billing a G0439, G0444-59, 99497-33, 99213-25, G2211 and 99406. My question is, do I need a modifier with the 99406 and if so, which one. I'm using F17.200 for diagnosis code if that makes any difference. Any help would be appreciated. Thanks.
 
holy smokes!! Your documentation is going to need to be very precise with regard to that preventive counseling.... The AWV does have an element of behavioral health risk assessment. This preventive counseling (not covered by Medicare) cannot be part of an E&M where you also cite time as a factor and--- separate from your ACP, where time is also a factor. If you're going to bill all of this on one claim, you'll need to be sure that your documentation for all of the codes is significant and separately identifiable, so that you're not unbundling. The 99406 would generally be reported with a -25 as a separately identifiable E&M service.
