Recently getting denials from BCBSNC for A1c (83036) billed at well child exam. Remark code: A04 - Max frequency limitation has been met for services reported. These have routinely been reimbursed with diagnosis code for obesity (E66.xxx) or the screening Z13.1. This just started a few weeks ago, no issues with Blue Card. I'm in a pediatric practice in NC. I can find no policy changes, just wondering if anyone else in NC having this issue?
Any thoughts?
