AAA g0389 cpt code getting denied for Z13.6 or Z 87.891 medicare needs a secondary diagnosis code which one is needed. they are denying cad or hypertension.

The Z13.6 is right as the encounter DX, but you need a secondary RISK factor diagnosis. You will need to check the LCD for your MAC, but the LCD in my area covers smoking and family history of as the two options. Family history meaning a 1st degree relative; smoking means more than 100 cigarettes in their lifetime. Z87.891 won't work because it isn't specific enough as to the status of smoking; it just indicates a history of smoking. The LCD here directs to the F17.2- nicotine dependence codes. If you plan on using the family history of DX, they will likely ask for documentation that shows the family hx of AAA had been documented.

Your MAC could have completely different rules, so I'd check there first.
