Absent Rash

Does anyone know the best way to bill an office visit where a patient presents with an "absent rash" - where the rash occurs off and on but isn't present at the visit? This would obviously mean we can't list a rash diagnosis code because it wasn't present. Do we bill the symptoms instead?
