Wiki Addendums for E&M visit

Can someone advise me where I can find it in black and white were you can ask for an addendum to an OV already signed by the provider to clarify a cheif complaint. My compliance person says I can send a compliance email and ask the provider to clarify what the cc is, because so often the provider will indicate the pt. is here to establsih care or to get his meds. I work for a VA hospital and the documentation to me does not support medical necessity for a visit and all our notes are done with a EMR, they also do a lot of copy and paste.
