Wiki Additional surgical fees for colonoscopy etc in the office?

Most insurance companies will provide higher reimbursement for colonoscopies performed in the office (POS 11) compared to in the hospital (POS 22). In these cases, I don't need to bill any additional codes or modifiers; the place of service is enough to trigger the higher payment.

However, some insurance companies do not give any such place-of-service differential. Are there any CPT codes that I can use to get reimbursed for the extra expenses incurred by the practice performing the procedure in the office?
