Hey folks...any thoughts??

POSTOPERATIVE DIAGNOSES:
1. Left ankle derangement.
2. Left ankle arthritis.

PROCEDURE PERFORMED: Left ankle arthroscopy with debridement and exostectomy.

** placement of the arthroscope
** noted to be hypertrophic synovitis and fibrosis to the ankle joint, both medially, laterally and anteriorly
** two loose bodies or exostosis noted within the anterior ankle joint
**remove them and sent them for pathologic evaluation
** small area medially on the talus that was denuded of cartilage
**This was then microfractured with the use of a microfracture awl.
** shaver debrided was then used to debride the remainder of the hypertrophic synovitis

M25.772 left ankle osteophyte
M19.072 left ankle arthritis
M65.872 left ankle synovitis
Can I code M93.972 for the denuded cartilage??

I am thinking 29891 for the microfx and 29898 for the debridement...

Any thoughts will be greatly appreciated

I agree with the CPT codes, but I think that M95.8 would better suit the denuded cartilage, because after looking it up, it sounds like a chondral defect, which would be the dx for the microfracture. I would also code M24.672 for the debrided fibrosis to better support the extensive debridement code. I hope this helps! :)
