Anthem Colorado BCBS has started denying screening mammograms with a diagnosis of z12.31. They are stating that z12.31 can't be the primary diagnosis code. What diagnosis code would be primary for screening mammogram cpt 77063, 77067 to get BCBS to pay the claim
