If one ear is instrumented, and the other is lavaged, how do we bill these procedures and dx?

It is going to depend on whether the ear wax was impacted in the ear where the wax was removed by lavage.

If the ear wax (cerumen) was impacted in both ears, then you would report:

  • 69210 with LT (or RT) modifier for the impacted cerumen removed by instrumentation
  • 69209 with RT (or LT) modifier for the impacted cerumen removed by lavage.

If the ear wax was not impacted, then either or both services are included in the E/M Service.
