I've come across this scenario a few times. What our office does is prior to this "consult" the party involved is told up front that this is strictly self pay? The visit is coded based on time as well.
We do not bill any insurance we bill the party who is coming in to discuss the medical issues/concerns etc....
Your office/facility may want to establish this thru compliance plan as well.
There used to be a code for billing w/o the patient present but if my memory serves me correctly most insurances did not pay for it anyway.
