Billing for codes 17270-17286

Everything that I've learned about billing these codes has said to add the sizes of all destructions for the same anatomical areas and bill to that size. So for 2 malignant lesions removed from these areas; scalp,neck,hands,feet, gentialia, etc; we would add the sizes together to find the correct code to bill.
I have a pathologist who is requesting documetation on this policy and I do not know where to look. The instructions in the standard edition CPT books we get offers no clear answer. Can someone point me in the right direction to look for something in writing that I can show him?

I don't think I have a copy of that. Would anyone know if this has been discussed in an issue of The Coding Edge or the EdgeBlast's? My resources are limited to the internet unfortunatly, unless someone would be willing to fax me a copy of anything in another publication.
