Wiki billing for LCSW

I'm new to billing for a LCSW or any behavioral Health. I read the article in Cutting edge and need some more guidance please. I need help explaining to the provider that the DX needs to be in each note. Am I correct in my thinking? She does not actually put the DX in her notes. Can anyone please help me so when I talk to her I'm not confused. Thank you for your help.

every claim needs a dx to be billed. if you dont have a dx, you cant bill. The dx is what supports the medicaly necessity of the visit.
also each and every note MUST stand alone.

The provider must indicate the reason for the patient encounter, they do NOT however have to include the dx code, just the diagnosis.
