I'm new to billing for a LCSW or any behavioral Health. I read the article in Cutting edge and need some more guidance please. I need help explaining to the provider that the DX needs to be in each note. Am I correct in my thinking? She does not actually put the DX in her notes. Can anyone please help me so when I talk to her I'm not confused:). Thank you for your help.

every claim needs a dx to be billed. if you dont have a dx, you cant bill. The dx is what supports the medicaly necessity of the visit.
also each and every note MUST stand alone.
