Wiki Billing for post op DME glasses and diagnosis requirements.

I have been thrown into a new position where I am filing the DME post op glasses claim. I can not get to any link or information on the required diagnosis codes that are covered. When we receive a referral from the surgeon, they are providing the RX needed for the post op glasses such as H52.13 for Myopoia Bilateral...and when our OD assess the patient he adds the code H53.8 for other visual disturbances for blurred vision. However, I am thinking the post op code is needed Z96.1 ...? Am I correct and do we need the referring physician to send over documentation for the post op with code Z96.1? or will H52.13 be suffice? I also wondered about the mod's RT and LT?
