1) If an NP under the provider's direct supervision see's a patient in follow up (not a new problem) is it ok (compliant) to bill the service using the provider (not the NP) as the billing and servicing provider?

2) If an NP sees a patient under direct supervision of the provider & wants to bill as incident to, does the provider have to document a separate note or can he/she simply co-sign the NPs note? If the provider co-signs the NPs note is it valid or compliant for the service to be billed out strictly under the provider's NPI or should the claim form have the NP listed as the "servicing provider" and the provider as the "billing provider"?

3) Where can I find (in writing) the rules and guidelines to such billing practices in reference to "incident to" billing?
 
Google MLN Matters article SE0441. Tells you everything you need to know as far as Medicare goes. Each payer has their own take on the process. Many don't allow it all all. You will have to enquire with each payer to determine their rules.
