I know that most insurances won't cover things like school physicals and pre employment exams, but are titers covered? We had a patient get Hep B titer done for school. Can we bill the patient's insurance or just bill the patient?

The health plan may deny the titer as a administrative/school exam related service depending on the policy's specific exclusions. However, it may be covered under the medical benefits because this is not a mandated wellness/preventive service under the ACA. The worst you can do is bill it and if the claim is denied then you bill the patient. Just be certain you are using the correct diagnosis to indicate that this test was performed for administrative purposes.
