HI Jackie,
36479 should not be billed without 36478 (it is an add on code), and 93971 and 76998 should not be billed at all with those codes. Look under the code(s) in the CPT book for more information.

As for modifers for 93971 and 76998 (in other coding scenarios) perhaps 26 and or 76 applies.
